To participate, you (or, if you are below the age of 18, your guardian) will need to sign both waiver forms.

You can submit the forms in either of these ways:

Print them out and bring them to the event.
Email signed or scanned copies directly to the coordinator at man@urbanforestfriends.org.

A family member or friend also wants to attend? They would need to submit a RSVP form as well! One RSVP per person, please!

How would you like to submit the waiver forms? * I will print them out physically and bring them to the event I will sign them digitally (or scan a copy) and send them to organizer at man@urbanforestfriends.org I have previous attended an UFF tree planting at Newark and have signed both forms in the calendar year of 2026. I do not need to sign again
